Instructions for completing the application

Download and save the
form
Click here to
download the on-line application
form to your computer. Or,
you can save the file directly to your computer, without opening it, by
right-clicking on the link itself and selecting "Save Target As" or
"Save Link As".
If you open the file directly in your browser save it to your
computer before you fill it in. We recommend that you save the
file, as that gives you the flexibility of of working on the application in
more than one session.
You will need Adobe Reader 7 or
later if you plan to save the filled-in file. Earlier versions of Adobe
Reader will allow you to save the blank form, fill it in, e-mail it, but
not to save it with the data filled in.
Fill in the form
When you are ready to work on the application form, click on the file
name to open it directly in Adobe Reader.
Check the “Highlight Fields” checkbox, which you should see at the top
of the document window. The form fields will then be highlighted.
Select the hand tool from the Reader toolbar menu.
Position the hand pointer inside a form field and click. The hand
pointer changes to an I-beam pointer, which allows you to enter text
from your keyboard. You can also select a check box in the same manner.
Press Tab to move to subsequent fields.
Press Shift + Tab to move back to previous fields.
After completing the application form or if you intend to finish it at
a later time, save it again to your hard drive or a removable disk or
drive.
Review the application form to be sure you have answered all the
questions. Review a printed copy and make any necessary revisions or
corrections.
If you wish to keep a filled-in copy of the application form for your
records, click on the “Print Form” button at the bottom of the last
page.
Submit the form
If you are working on your own computer, you can submit the completed
Application form by using the “Submit Form by email ” button at the
bottom of the last page. Be sure to attach your
application essays
to the e-mail as a Word document or pdf file.
If you are working at a public computer (library or school, for
instance), it is suggested that you submit the completed form by a
web-based e-mail program (such as Yahoo, Gmail or Hotmail). If you do
not have a web-based email program available print out and send the
filled-in application form by U.S. Mail or by fax.
You should consider deleting the filled-in application form from a
public computer after you have submitted it and/or printed it out. This
will ensure that your privacy is maintained.

Follow the instructions above for downloading the form
or use the form at the back of the printed catalog.
Complete the form by hand and be sure that your handwriting is
legible.
